• Nutrition, physical activity or mental health • Tailored health and wellbeing groups • Linkages with health and wellbeing services

Our team of mental health professionals provide assessment, evidence-based interventions and therapeutic services to adults and children who either have a diagnosed mental health condition or are at significant risk of developing a mental health condition. Services are provided across a Stepped Model of Care, ensuring that the most appropriate service is provided to each client at the right time. FocusOne Health work closely with General Practice and other key services to link clients with services outside of FocusOne Health if appropriate. The Integrated Wellbeing Service (IWS) provides care coordination and support for patients with chronic disease to better manage their conditions and tackle risk behaviours that may impact their wellbeing. IWS has a strong focus on promoting healthy lifestyle behaviours and building a better understanding of chronic disease through illness prevention initiatives for patients and community. Please click here to view flyer.

Supporting people to develop the confidence and skills to participate in learning and to navigate a path to education or employment. We provide health services to young people 12-25 years, their families and their friends across the Riverland. headspace Berri is about early intervention and support for, mental health, general health, drug and alcohol issues and employment and education support. Referrals to our services are accessed by phone, dropping in or via a referral form. Visit the headspace Berri website here.

Improving health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions through care coordination services.
