Become a FocusOne Health Member

Becoming a member provides a range of benefits for primary health care workers in the Riverland Region.

HOW DO YOU BECOME A MEMBER?

Members are welcome from across the primary health sector who conduct their services in the Riverland and meet the following membership categories:

Ordinary Membership shall be open to General Practitioners and will have full voting rights.

  • Associate Membership shall be open to all GP Registrars, Interns and medical students during their term of placement within the Riverland. Associate members do not have voting rights
  • Practice Membership shall be open to any General Practice operating within the Riverland and enables all current employees, including Practice Managers, Practice Nurses, Reception/Administration staff and Allied Health Professionals, to hold a membership. Practice members do not have voting rights
  • Medical Specialist Membership shall be open to any Medical Specialist who conduct their clinical practice in the Riverland. Medical Specialist Membership do not have voting rights
  • Allied Health Professional Membership shall be open to Allied Health Professionals who conduct their practice or are employed by a health provider in the Riverland. Allied Health Professional Membership do not have voting rights. Allied Health Professional Membership will automatically cease when a member concludes conducting their clinical practice in the Riverland
  • Honorary Membership shall be available to any Medical Practitioner residing or practicing within the boundaries of RDGP.  Honorary members do not have voting rights. Such membership shall be bestowed by the Board of Management.

Our Services

Healthy Habits
A care coordination program for all ages that supports the development of long-term sustainable healthy habits, through the development of a care plan. Healthy Habits Care Coordinators (RN’s) walk side-by-side clients to identify the health factors they need support with; to create a healthy lifestyle – this may include:
• Nutrition • Mental health • Physical activity • Tailored health and wellbeing groups • Community programs, such as walking groups • Linkages with health and wellbeing services, such as general practice

Foundation Skills Program
Supporting people to develop the confidence and skills to participate in learning and to navigate a path to education or employment. headspace Berri
We provide health services to young people 12-25 years, their families and their friends across the Riverland. headspace Berri is about early intervention and support for, mental health, general health, drug and alcohol issues and employment and education support. Referrals to our services are accessed by phone, dropping in or via a referral form. Visit the headspace Berri website here.
Closing The Gap Riverland (CTGR)
Improving health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions through care coordination services. Limestone Coast Services Directory
The Country SA Services Directory website is designed to quickly identify availability of services across the Riverland, Mallee and Limestone Coast. The concept of the online services directory is to provide communities with current information about the health, community and support services available in their region. The directory also has a number of online resources and telephone services listed to assist people to find the right service at the right time to meet their needs. Riverland Services Directory

Mental Health
Our team of mental health professionals provide assessment, evidence-based interventions and therapeutic services to adults and children who either have a diagnosed mental health condition or are at significant risk of developing a mental health condition. Services are provided across a Stepped Model of Care, ensuring that the most appropriate service is provided to each client at the right time. FocusOne Health work closely with General Practice and other key services to link clients with services outside of FocusOne Health if appropriate. The team offer a range of 1:1 and group therapy services for the community across the Riverland, Limestone Coast & part of the Mallee sub-region (Pinnaroo). To view our mental health programs and services, please click here.
Integrated Wellbeing Service

The Integrated Wellbeing Service (IWS) provides care coordination and support for patients with chronic disease to better manage their conditions and tackle risk behaviours that may impact their wellbeing. IWS has a strong focus on promoting healthy lifestyle behaviours and building a better understanding of chronic disease through illness prevention initiatives for patients and community. Please click here to view flyer.
