Mental Health

Riverland, Limestone Coast & PInnaroo

Our team of mental health professionals provide assessment, evidence-based interventions, and therapeutic services to adults and children who either have a diagnosed mental health condition or are at significant risk of developing a mental health condition. Services Offered Include:

Our program is dedicated to providing accessible and comprehensive mental health support to the Mount Gambier community, including information, assessments, support or referrals. We connect individuals to services that meet their unique needs, fostering strong connections and providing essential service navigation.

Integrated Primary Care (IPC)

Limestone Coast & Mallee

A program that supports areas of communities under 8000 to access allied health and specialist nursing services. Working closely with general practice and allied health providers, IWS aims to provide care coordination and support for patients with chronic disease to better manage their conditions and tackle risk behaviours that may impact their wellbeing.

headspace Berri

Riverland

We provide health services to young people 12 – 25 years, their families and friends across the Riverland. headspace Berri is about early intervention and support for mental health, general health, drug and alcohol issues and employment and education support. Referrals to our service are accessed by phone, dropping in or via a referral form.

Foundation Skills

Riverland

Foundation Skills build the health literacy of the Riverland community. A key component of this program is increasing the foundation skills, communication skills, literacy and numeracy skills of participants to better equip them to pursue community participation, study and/or career goals. Programs are interactive and delivered in a safe, informal and social environment.

Closing the Gap Riverland

Riverland

A service tailored for Aboriginal and Torres Strait Islander people, our team of nurses support chronic disease management by coordinating access to primary health care services. CTGR is focused on the promotion of healthy lifestyles for all ages and management of chronic disease; care plans are developed and implemented to support individual health goals, provide service linkages and encourage self management.

Healthy Habits

Riverland, Limestone Coast & Murray Bridge

Healthy Habits is a wellbeing program that supports the development of long-term sustainable healthy habits. Healthy Habits Care Coordinators walk along side people to identify the health factors they need support with; in order to create a healthy lifestyle and prevent the on-set of chronic disease.

Care Finder

Riverland & Limestone Coast

The care finder program provides short-term intensive support to help understand your options and access aged care services and other supports in your community.

If you are experiencing barriers to accessing My Aged Care services, our care finders can work with you. Care finders can help you connect with eligibility screening, attend appointments with you if it’s appropriate, help you understand the process, find and connect with the right services, and check in with you now and then to make sure the services are meeting your needs.

Bereavement Navigation Service

Statewide

The Bereavement Navigation Service helps people of all ages connect with bereavement and grief information and available support. Services are available via telephone, telehealth, and in person across Adelaide metro, Riverland, Limestone Coast, Whyalla, and Port Augusta. To speak to a Bereavement Navigator, call 1800 725548 (PALLI8) or (08) 8472 8255.

The Bereavement Navigation Service is part of Palliative Care Connect. The pilot is funded by the Australian Government Department of Health and Aged Care with support from SA Health.

FocusOne Coach

Low Intensity Cognitive Behavioral Therapy

You will benefit from coaching with a suitably qualified Low-Intensity Cognitive Behavioural Therapy (LiCBT) Practitioner and will receive a workbook specific to your problem or concern. You will meet with the LiCBT Practitioner weekly for up to 8 weeks. LiCBT services are available to community members in the Riverland and Limestone Coast.
